Yanchep Midwives (“Yanchep Midwives”, “we”, “us”, “our”) is committed to protecting the privacy and confidentiality of personal information, including health information, in accordance with the Privacy Act 1988 (Cth) and the Australian Privacy Principles.
This Privacy Policy applies to services provided through Yanchep Midwives, including the Yanchep Feeding Clinic. It explains how we collect, use, store and disclose personal information, and how you may access or correct information we hold about you.
2. What Information We Collect
We may collect personal information including your name, date of birth, address, email address, telephone number, Medicare and other relevant healthcare information, pregnancy, birth and postnatal information, infant feeding information where relevant, medical and health history, current health concerns, medications and supplements, allergies or sensitivities, relevant family health history, information regarding your baby where relevant to the care being provided, appointment and billing information, and correspondence or information received from other healthcare professionals or services.
We only collect information that is reasonably necessary to provide safe and appropriate care and to operate our practice.
3. How We Collect Information
Personal and health information may be collected directly from you through enquiry, registration and intake forms, online booking systems, consultations, telephone, SMS or email communication, information provided through our website, clinical assessments, and information provided by another healthcare professional or service involved in your care.
Where appropriate, information may also be received from hospitals, general practitioners, obstetricians, pathology providers, imaging providers and other healthcare professionals involved in your care.
4. Purpose of Collection
We collect and use personal information to provide midwifery, postnatal and infant feeding care; assess, plan and review your care; maintain an appropriate clinical health record; communicate with you regarding appointments and your care; coordinate care with other healthcare professionals and services; arrange or receive referrals, pathology, imaging and other clinical information; process Medicare claims and billing where applicable; manage administrative and practice operations; and meet professional, legal and regulatory obligations.
5. Use and Disclosure of Information
Your personal and health information will generally only be used or disclosed for the purpose for which it was collected or for a directly related purpose that would reasonably be expected in the provision of your healthcare.
Information may be shared with healthcare professionals or services involved in your care where appropriate, including your GP, obstetric or maternity care providers, hospitals, pathology providers, diagnostic imaging providers, allied health practitioners and other healthcare professionals involved in your care.
Information may also be disclosed where you have provided consent, where disclosure is required or authorised by law, where necessary to prevent or lessen a serious threat to health or safety, or where otherwise permitted under applicable privacy legislation.
Yanchep Midwives does not sell, rent or trade personal information. Personal information will not be used for marketing without appropriate consent.
6. Electronic Systems and Third-Party Providers
Yanchep Midwives may use secure third-party systems to assist with appointments, clinical records, communications, Medicare claiming and practice administration. These may include online booking systems, practice management software and secure electronic communication platforms.
We take reasonable steps to use reputable providers and maintain appropriate security arrangements when personal and health information is stored or transmitted electronically.

We take reasonable steps to protect personal information from misuse, interference, loss, unauthorised access, modification or disclosure. Health records are retained in accordance with applicable professional, legal and regulatory requirements.

Where practicable, you may make a general enquiry anonymously or using a pseudonym. However, identification will generally be required when receiving clinical care, where Medicare services are being provided, or where identification is otherwise necessary to provide safe and appropriate healthcare.
9. Access and Correction
You may request access to personal information held about you or ask us to correct information that is inaccurate, incomplete or out of date. Requests can be made by contacting Yanchep Midwives. We will respond within a reasonable timeframe and in accordance with applicable privacy legislation.
10. Complaints and Concerns
If you have concerns about how your personal information has been handled, please contact us in writing. We take privacy complaints seriously and will investigate and respond within a reasonable timeframe.
