Yanchep Midwives

1. Agreement
By booking, accessing or receiving services through Yanchep Midwives, including Yanchep Feeding Clinic, you agree to these Terms and Conditions. These Terms apply to services provided through the practice unless separate written terms or agreements apply to a particular service.
2. Scope of Services
Yanchep Midwives provides private midwifery and related healthcare services. Depending on the service booked, this may include antenatal midwifery care, postnatal midwifery care, infant feeding assessment and support, breastfeeding support, education and guidance, and referral or coordination with other healthcare providers where appropriate.
The Yanchep Feeding Clinic provides assessment, education and support relating to infant feeding within the scope of practice of the treating practitioner. The exact scope of care will depend upon the service booked and the individual client’s needs.
3. Independent Practitioners
Healthcare professionals providing services through Yanchep Midwives may practise as independent practitioners. Each practitioner remains individually responsible for the clinical care they provide and practises within their own professional scope, registration, competence and professional indemnity arrangements.
Where care is provided by more than one practitioner, relevant clinical information may be shared within the practice where reasonably necessary for continuity and safety of care.
4. Clinical Care and Limitations
Yanchep Midwives provides professional midwifery and infant feeding care within the scope of practice of the treating practitioner. Clinical assessment and recommendations are based on the information available at the time of consultation. Healthcare outcomes cannot be guaranteed.
Where a concern falls outside the scope of the treating practitioner, you may be advised to seek assessment or treatment from another healthcare provider.
5. Emergency and Urgent Care
Yanchep Midwives and Yanchep Feeding Clinic are not emergency services. If you or your baby require urgent medical attention, you should seek appropriate urgent or emergency healthcare. In an emergency, call 000. Depending upon the circumstances, you may also be advised to contact your maternity hospital, GP, emergency department or other relevant healthcare provider.
Electronic messages, emails and routine practice telephone numbers should not be relied upon for emergency medical care.
6. Appointments and Practitioner Availability
Appointments are subject to practitioner availability. If your scheduled practitioner becomes unavailable because of illness, leave or unforeseen circumstances, we may offer an appointment with another appropriately qualified practitioner or a rescheduled appointment.
Where a particular practitioner is unable to provide ongoing care, appropriate alternative arrangements or referrals will be discussed where possible.
7. Information You Provide
You agree to provide information that is accurate and complete to the best of your knowledge. Please advise your practitioner if relevant information changes or if you become aware that information previously provided was incorrect. Safe clinical care relies upon practitioners having access to accurate and current information.
8. Privacy and Health Records
Personal and health information is collected, used and stored in accordance with the Yanchep Midwives Privacy Policy. Clinical records may include information provided through online booking and registration forms, consultations, correspondence and information received from other healthcare providers.
9. Medicare and Fees
Some services provided through Yanchep Midwives or Yanchep Feeding Clinic may be eligible for Medicare benefits where applicable. Other services may attract a private fee.
Where a fee applies, this will be communicated at the time of booking or before the service is provided wherever reasonably practicable. Clients remain responsible for fees associated with services that are not covered by Medicare or another funding arrangement.
10. Cancellations and Changes
Appointment cancellation or rescheduling requirements may vary depending upon the service booked. Any applicable cancellation fees or booking conditions will be communicated through the booking process or at the time an appointment is made.
11. Website and Online Information
Information provided on the Yanchep Midwives website, social media pages and other general educational materials is intended for general information and education. It does not replace an individual clinical assessment.
If you have concerns regarding your own health or your baby’s health, appropriate individual healthcare assessment should be sought.
12. Changes to These Terms
Yanchep Midwives may update these Terms and Conditions from time to time. The most current version will be available on the Yanchep Midwives website.
